DOES VAPING CAUSE ACNE? WHAT THE RESEARCH ACTUALLY SAYS

The short answer is: there isn’t enough evidence to say that vaping directly causes acne. However, that doesn’t mean vaping and your skin are completely unrelated.

If you’ve started vaping and suddenly your face looks like it has declared war on you, it’s understandable to ask: does vaping cause acne?

The short answer is: there isn’t enough evidence to say that vaping directly causes acne. However, that doesn’t mean vaping and your skin are completely unrelated.

Some components of e-cigarette aerosol, including nicotine and other chemicals, can affect biological processes linked to inflammation and oxidative stress. Laboratory research has also found that exposure to vape liquids and aerosol residues can trigger inflammatory responses in human skin models.

So, as I see it, the honest answer is somewhere between “vaping definitely causes pimples” and “vaping has absolutely nothing to do with your skin.”

What Actually Causes Acne?

Before blaming your vape, it helps to understand what acne is.

According to the American Academy of Dermatology (AAD), acne develops when hair follicles, or pores, become clogged. Excess oil, dead skin cells and inflammation all play a role. Hormonal changes can also increase oil production and make breakouts more likely.

That gives us the basic acne equation:

Excess oil + dead skin cells + clogged pores + inflammation = acne

And unfortunately, your skin doesn't need much encouragement to join the party.

Stress, lack of sleep, certain skin-care or hair-care products, hormones and other environmental factors can also make acne worse.

Can Vape Chemicals Cause Skin Inflammation?

This is where the science gets more interesting.

A 2021 study published in Toxicology in Vitro investigated e-cigarette refill fluids and aerosol residues using cultured human keratinocytes and a three-dimensional human skin model.

Researchers found that some e-cigarette fluids increased reactive oxygen species (ROS) and inflammatory responses. Exposure was associated with increased secretion of inflammatory markers including IL-1α, IL-6 and MMP-9. The researchers concluded that chemicals from e-cigarettes could cause oxidative stress and inflammation in human skin models.

But there is a very important asterisk here.

A laboratory skin model is not the same thing as a person developing acne from vaping.

The study demonstrates a biological mechanism worth investigating. It does not prove that vaping causes acne in humans.

That's an important distinction that often gets lost when scientific findings are turned into internet headlines.

Can Vaping Make Existing Acne Worse?

Possibly, but we don't currently have enough human evidence to put a number on the risk.

If vaping contributes to oxidative stress or inflammation, it is biologically plausible that it could aggravate certain inflammatory skin conditions in susceptible people. But “biologically plausible” isn't the same as “clinically proven.”

If your acne started or became noticeably worse around the same time you began vaping, I'd look at the bigger picture rather than immediately blaming the vape.

Ask yourself:

  • Did you recently start vaping?
  • Did you change your nicotine strength?
  • Are you sleeping less?
  • Has your stress level increased?
  • Have you changed your diet?
  • Did you start using new skin-care products?
  • Are you touching your face more often?
  • Have you changed your shaving routine?
  • Are you exercising more and sweating more?
  • Did your acne appear around a hormonal change?

Sometimes the culprit isn't the shiny little device sitting in your hand. It's something much more boring.

What Should You Do If You Think Vaping Is Causing Your Acne?

If you suspect a connection, the most useful approach is to look for patterns rather than panic.

1. Keep your vape clean

Don't let a device covered in e-liquid, dust and fingerprints become part of your skincare routine.

Clean the exterior according to the manufacturer's instructions and avoid getting e-liquid on your skin.

2. Stop touching your face

This is easier said than done.

If you're constantly holding your vape, you may be touching your mouth and face more frequently. Wash your hands regularly, particularly before touching your face.

3. Keep your skincare routine simple

The AAD recommends acne-friendly skincare and appropriate treatments depending on the type of acne. Products labeled oil-free, non-comedogenic or non-acnegenic can be useful for acne-prone skin.

You don't need a 14-step skincare routine worthy of a laboratory.

4. Don't aggressively scrub your skin

Scrubbing harder doesn't mean acne disappears faster.

Irritating already-inflamed skin can make things worse. A consistent, gentle routine is generally a better starting point.

5. Look at your sleep and stress

The AAD notes that stress can worsen existing acne, while inadequate sleep may also contribute to breakouts.

If you've been vaping more because you're stressed, sleeping badly and eating differently, the vape may be only one piece of the puzzle.

6. Consider talking to a dermatologist

If acne is painful, persistent, severe or leaving scars, don't spend months trying to solve the mystery yourself.

A dermatologist can determine whether you're actually dealing with acne and recommend treatment based on the type and severity of your breakouts.

Conclusion

The current evidence doesn't establish vaping as a direct cause of acne. However, laboratory research suggests that some e-cigarette chemicals can promote oxidative stress and inflammatory responses in human skin models.

If you've noticed breakouts after starting vaping, pay attention to the timing, your nicotine use, skincare routine, stress, sleep and other lifestyle changes.

Your vape may be part of the story. It just isn't fair to hand it the entire script yet.
